Healthcare Business Books Cleanup Which form carries the pay

The form follows how the person was paid. A medical payment and a nonemployee service are not interchangeable.

Box 6 is not the nonemployee form

Payments for medical and health care services are reported on Form 1099-MISC. The IRS information-return page places those payments in box 6 and ties them to the reporting threshold. That is a different form from the one used for nonemployee services. An associate clinician paid as a nonemployee belongs on the form that matches how the person was paid, not on whichever form the software opened first.

For payments made in 2026, nonemployee services are measured against $2,000 on Form 1099-NEC. The instructions raise the threshold on certain information returns to $2,000 for tax years beginning after 2025. Cash paid in 2025 is still tested against the older $600 line. Using the new line on an old year, or the old line on the new year, is a common way to file the wrong count. Backup withholding is reported at any amount, including amounts under the threshold. Public Law 119-21, at section 70433, is what moved the newer nonemployee line. The page to open beside the form is the IRS information-return page.

Typing, outside clinicians, and the clinic's own staff

Someone paid to type a dictated note is a payee, not a product and not a visit. If that person is an employee, wages run through the payroll company the clinic already uses, and this practice does not take payday over. If that person is paid as a nonemployee, the information return is the one the instructions name for that kind of pay, at the threshold for the year of the payment.

A W-9 collected in March does not fix a payment that was made last November with no taxpayer identification number on file. The cleanup looks at the year the cash moved. It also looks at backup withholding when the identification was missing. We do not promise a form count from a list of names with no amounts beside them.

What we need before we say which form

We need the payee's name, whether the person was an employee or not, the year the cash moved, and the amount. With those four facts the instructions do the sorting. Without the amount, we can tell you which form family is likely, and we will not tell you that a filing is required.

Staff who are already on payroll are not re-sorted onto a 1099 because a January got busy. Moving people between those two worlds is a classification question with consequences, and it is not a coding shortcut. The setup of payroll, as distinct from running it, is on pay setup.
